Information you give us
The only information this site asks you for is what you enter in the free case review form. That form collects four things:
- Your first name
- Your phone number
- The state you are in
- Your relationship to the injured person
We do not ask for your email address, your postal address, your date of birth, or any financial information. We do not ask you to upload documents or medical records. If you type additional details into a field, we receive whatever you type, so please do not send sensitive information you would not want shared.
Information collected automatically
Like most websites, this site records technical information when you visit: your IP address, browser and device type, the pages you view, how long you spend on them, and the site or search you arrived from. This comes from three sources:
- Google Analytics and Google Tag Manager, which we use to understand which pages people find useful. These set cookies in your browser.
- Google reCAPTCHA, which runs on the case review form to block automated spam. Google receives your IP address and interaction data when the form loads. Its use is governed by Google’s own privacy policy and terms of service.
- Our web host and security plugin, which keep server and security logs, including IP addresses, in order to keep the site online and block attacks.
How the information is used
Information you submit through the form is used for one purpose: to connect you with an independent attorney who handles nursing home abuse and neglect matters in your state. Automatically collected information is used to operate the site, keep it secure, and measure which pages are working.
We do not sell your personal information, and we do not use it for advertising targeted at you.
Who your information is shared with
When you submit the case review form, your submission is:
- stored in this website’s database;
- emailed to the people who operate this site;
- passed to an automation service (Zapier) that routes new submissions; and
- shared with an independent attorney for review.
We also rely on ordinary service providers who necessarily handle data in the course of running a website: our web hosting company, Google (analytics and reCAPTCHA), and our security plugin provider. We may disclose information if we are legally required to, or where it is necessary to protect someone’s safety or our legal rights.
Submitting the form does not make you a client
This matters more than anything else on this page. Submitting the form does not create an attorney-client relationship with National Nursing Home Lawyers or with any listed firm, and what you send is not protected by attorney-client privilege.
